NOTICE REGARDING INVESTIGATIVE CONSUMER REPORT

As part of your rental application, an investigative consumer report may be obtained about you. This report may include information about your character, general reputation, personal characteristics, and mode of living. This information will be obtained through personal interviews with sources such as your neighbors, friends, associates, former landlords, or others with whom you are acquainted.

YOUR RIGHTS:

• You have the right to request, within a reasonable time, disclosure of the nature and scope of the investigation
• You have the right to request and receive, within 3 business days of your request or within 3 business days of the report being obtained (whichever is later), a complete and accurate copy of the investigative consumer report
• You may request that the report include the names and addresses of all sources contacted, if the report contains public record information
